The unilateral deep inferior epigastric perforator flap: comparing university to community hospital.

چکیده
The deep inferior epigastric perforator (DIEP) flap is considered to be the gold standard for autologous breast reconstruction. This study evaluates the outcome of unilateral DIEP flap reconstructions, comparing university with a community hospital setting. A total of 77 unilateral DIEP flaps were performed at one university hospital and two community hospitals by the same two surgeons. Outcome parameters were: hospital stay, operating time, wound infection, wound dehiscence, fat necrosis, haematoma, (partial) flap necrosis and the need for surgical intervention. Forty-nine unilateral DIEP flaps were performed in the university hospital and 28 in the community hospitals. Baseline characteristics were equal. No significant difference was found in total complication rate, flap loss or need for surgical intervention. Although wound dehiscence occurred more often in the community hospitals, unilateral DIEP flap breast reconstructions can be performed with a comparable degree of safety and complication risk in both university and community hospital settings.

The Utility of Computed Tomographic Angiography in Preoperative Planning for Deep Inferior Epigastric Artery Flap Breast Reconstruction
Introduction: Abdominal donor site flaps are the standard for autologous breast reconstruction. Because of extreme variations of abdominal wall vascular anatomy between individuals, and even two hemi-abdomens of the same person, it seems valuable to use a sensitive method for preoperative evaluation. In this study, we evaluated the effects of preoperative abdominal wall computed tomographic ang...
متن کاملTransplantation in identical twins: another option for breast reconstruction.
BACKGROUND Deriving their name from the Greek word plastikos, meaning related to molding, plastic surgeons have made their reputation by transplanting tissues from one area of the body to another to "remold" defects caused by trauma, congenital anomaly, or disease. Dr. Joseph Murray, a plastic surgeon, performed the first successful kidney transplant between identical twins in 1954. The authors...

Breast reconstruction using a perforator-based autologous flap is increasingly recognized as the preferred technique that provides an aesthetically pleasing restoration of breast form and volumewith longevity. Autologous flap reconstruction is widely accepted as the gold standard with deep inferior epigastric artery perforator (DIEP) flap being favored.1 When there is insufficient abdominal tis...

متن کاملThe Impact of Perforator Number on Deep Inferior Epigastric Perforator Flap Breast Reconstruction
BACKGROUND Perforator flaps minimize abdominal site morbidity during autologous breast reconstruction. The purpose of this study was to assess whether the number of perforators harvested influences the overall deep inferior epigastric perforator (DIEP) flap survival and flap-related complications. METHODS A retrospective review was performed of all DIEP flaps performed at the Hospital of the ...
